What is PCOD?

Polycystic ovarian disease (PCOD) is a very common hormonal disorder among women who have reached and are of reproductive age. It is a condition in which imbalance in hormones affects follicular growth during the ovarian cycle. This causes the affected follicles to remain in the ovary itself. If you are suffering from PCOD, then your pituitary gland may release abnormally high amounts of luteinizing hormone (LH) into your bloodstream. This disrupts your normal menstrual cycle and as a result, your follicle does not mature and therefore, ovulation does not occur. This is a major cause of infertility among women.

In addition, your blood will record high levels of insulin. These high levels of insulin coupled with increasing levels of LH or luteinizing hormone can then lead to excess production testosterone.  The high level of testosterone is what prevents ovulation causing infertility. Excess hair growth, acne and obesity, infrequent or prolonged menstrual periods, are common in women with polycystic ovary syndrome. In adolescents, infrequent or absent menstruation is a warning sign of the development of PCOS. In women who are past adolescence, difficulties in becoming pregnant or sudden unexplained weight gain may be the first sign of this condition. Ayurveda’s view on PCOD.

In Ayurveda, the state of balance of doshas is the main component responsible for health and any disruption to this state will lead to disease. This dosha-vaishamya is directly connected to symptoms of diseases and the relation between doshas and lakshanas (symptoms) are permanent.
Following the outlook of the symptoms of PCOS, as per modern description, it is clear that even though the symptoms are not compiled as a syndrome in Ayurveda, most of them have been described as features of separate diseases or conditions.

The symptoms that are commonly found in these conditions can be summarized as below-

  1. Menstrual irregularities are described under artava vyapads or Yonirogas (uterine disorders).
  2. Anovulation is categorized under Vandhya (infertility).
  3. Obesity is described as Sthoulya, a santanpanajanya vikara.
  4. Acne and Baldness are categorized as Mukhadooshika and Khalitya, two independent pathogenesis.
  5. Hyperinsulinemia leads to type 2 Diabetes mellitus, and is called prameha. It also manifests itself as a complication of sthoulya.

Since menstrual irregularities including anovulation and obesity are the most commonly visible symptoms these two have to be attended to immediately.

Pathology or origin of the diseased condition:

When the unbalanced vata etc. impairs the mamsa, shonita and meda mixed up with Kapha;  they produce these circulars, knotted and raised inflammatory swelling called ‘Granthi’. This type of glandular swelling is comparable to the modern terminology of a ‘cyst’. Cyst is an abnormal closed epithelium- lined cavity in the body which contains liquid or semisolid material.

In PCOS, development of follicles has been arrested or is dormant at one or any level and remains as it is. The cysts are follicles at varying stages of maturation and atresia. So, these cysts are not destined to reach the ovum. Thus, this pathology is compared with ‘granthibhuta artava dushti’  i.e. ‘cyst’. This is because, in PCOS, the follicles become cysts instead of developing into the mature ovum.
