Types of PCOS

For understanding different types of PCOS, one must understand the types of phenotypes. It is used for describing the composition or characteristics of PCOS. Based on this, the patients can be categorized into different phenotypes in three different criteria. This includes

  • Hyperandrogenism, either biochemical or chemical
  • Polycystic ovarian morphology on imaging
  • Ovulatory dysfunction

Based on the above three criteria, the patients are divided into four groups which include.

  1. Phenotype A (HA+ OD+ PCOM)

It is a full-blown PCOS wherein the patient shows pronounced menstrual dysfunction, high level of insulin, obesity, metabolic syndrome, and other concerning forms of dyslipidemia. Women suffering from this type of PCOS have high anti-Mullerian hormone levels resulting from an increase in the ovarian follicles. However, despite the increase in the follicles, women generally experience infertility as they won't ovulate naturally.

  1. Phenotype B (HA+OD)

It is a non-polycystic ovary insulin resistance. The PCOS symptoms in the patients are clinically hyperandrogenism resulting in biochemical causing the ovulatory disturbance without any visible polycystic ovaries on the ultrasound. Similar to phenotype A the women here experience irregular periods, insulin resistance, increased insulin, metabolic syndrome, and obesity. Herein AMH level can be high or normal, but a patient still suffers from infertility as they have extreme menstrual irregularities.

  1. Phenotype C (HA+PCOM)

It is the type of PCOS that makes the women experience an intermediate level of androgens. Thus they don't suffer from severe metabolic syndrome or hirsutism. However, the AMH level herein is much lower, which results in decreased ovarian follicles, causing infertility.

  1. Phenotype D (OD+PCOM)

It is known as non-hyperandrogenic PCOS. Women with this type of PCOS experience the mildest degree of metabolic and endocrine dysfunction. Also, there is less chance of obesity, metabolic syndrome, hyperandrogenism, insulin resistance, etc. But it is vital to know that this type of PCOS might lead to an increased number of follicles along with ovulatory dysfunction resulting in infertility.

This type of PCOS must be diagnosed on time to decide the right therapy and PCOS treatment. Although there is no permanent cure for this issue, the condition can be controlled when identified during the early stages.
